This PalmPilot™/ Palm medical software form was designed and programmed for a major university hospital.  The patient name, physician code, dates, and medical record numbers are entered by the user, who simply selects the diagnosis for the patient.  This displays a drop down menu.  When the treatment menu item (s) are selected the billing code number is recorded.
