This is a very common PalmPilot™ medical health application used in most hospitals.  It is used to track the status of patients with respiratory problems or a history of respiratory problems. The doctor or nurse fills in the variable information and then uses check boxes, radio buttons, and drop down menus to quickly complete the form.  Detail areas are accessed by tapping the "paper icons" to access  related forms.
